Referrals To Our Service
Who We Support
We work with adults affected by acquired brain injury (ABI), which can include injuries caused by trauma, stroke, infection, surgery, or other medical events. Our support services aim to help people rebuild their confidence, independence, and quality of life.
We accept referrals from:
GPs and NHS professionals
Social workers and case managers
Voluntary and community sector organisations
Families, carers, and individuals directly
If you're not sure whether someone is eligible, we encourage you to contact us — we're always happy to talk through individual circumstances.
Our Referral Criteria
To ensure we can provide the best possible support, we do have some criteria in place.
We can consider referrals for people:
Aged 18 and over
Living in Derbyshire or East Staffordshire
Who have a diagnosed or suspected acquired brain injury
Who are seeking to engage in positive support and rehabilitation
We are unable to accept referrals where:
The individual is currently experiencing active addiction issues
The primary diagnosis is a mental health condition that is not being treated or managed
There is ongoing high risk that would prevent safe engagement with our service
All referrals are reviewed on a case-by-case basis and final decisions are made at our discretion. Our priority is to ensure both the safety of the individual and the suitability of our support for their needs.
How to Make a Referral
You can refer someone using our Referral Form, which can be accessed by clicking the link below.
Once we receive a referral, our team will:
Acknowledge the referral at our next referrals meeting (these take place every Friday)
Contact the referrer (and/or the individual referred) for more information if needed
Assess the referral to determine suitability
Arrange an initial meeting if appropriate
